Compounded medications

If your provider prescribes a compounded medication, you understand that compounded medications are prepared by state-licensed pharmacies but are not FDA approved, and that the FDA does not verify their safety, effectiveness, or quality before they are dispensed. Your provider will discuss whether a compounded medication is appropriate for you.

Your responsibilities
