Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. We are required by law to maintain the privacy of your protected health information, to provide you with this notice, and to abide by its terms.

We use and disclose your health information for treatment (sharing your intake with the provider reviewing your case and the pharmacy filling your prescription), for payment (processing charges for your plan), and for healthcare operations (quality improvement, safety monitoring, and care coordination).

We may also disclose information when required by law, to prevent a serious threat to health or safety, or for public-health activities such as adverse-event reporting.

Uses and disclosures not described in this notice — including most uses for marketing and any sale of your health information — will be made only with your written authorization. You may revoke an authorization at any time, in writing, except to the extent we have already relied on it.

You have the right to inspect and receive a copy of your health records, to request corrections, to request restrictions on certain uses and disclosures, to request confidential communications, and to receive an accounting of certain disclosures. Requests can be made through your patient portal or by emailing hello@clinvi.com.

We reserve the right to change this notice and to make the revised notice effective for information we already hold. The current version, with its effective date, is always available on this page.

If you believe your privacy rights have been violated, you may file a complaint with us at hello@clinvi.com or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
